Dear God,

I know that I am a sinner and do not deserve Eternal Life. I also know that there is nothing that I can do about my sinful condition on my own. I believe that You sent your Son, Jesus Christ, to re-establish my eternal relationship with You. I believe that Jesus died for my sins, and rose from the grave so that I might have Eternal Life. I believe that He has purchased a place for me in Your Kingdom.

Right now, I sincerely regret my sins, and I turn away from my sinful ways and turn toward You. I ask You, God, to forgive me of my sins.

Lord Jesus, I ask You come into my heart, take control of my life and save me from the penalty of sin. By faith I accept You as my Lord and Savior. By faith I am trusting in You, and You alone, to restore my eternal relationship with God.

Holy Spirit, come into my life, transform my mind and heart, and help me to become all that God desires me to be.  It is in Jesus' name that I pray, Amen.
